Forms for Surgery, Appointments, and End of Life Services

To save you time before your visit or during drop off, please fill out the correct form(s) below. These forms are also available to be filled out at our facility; however, please give an additional ~15 minutes to complete them.


Admission Form (new patient/pets):

This form is filled out when you are a new client with Porter Pet Hospital or if you are an existing client bringing in a new patient.  This admission form must be filled out in it's entirety.  Please provide an e-mail if possible so we can better communicate with you more quickly and after hours if needed. Please print and complete the Admission Form below:

Euthanasia Form:

If you are a new client with us or this is a new pet we have not seen before, you must also fill out the Admission Form.  Please call the office for questions about cost of services such as private cremation, paw prints and anything else we can do for you during this time. These appointments are conducted in our private comfort room. Do not hesitate to call with questions or concerns prior to your appointment at 440-777-0888. The release form below can be printed and completed prior to your appointment or at our clinic on the day of service.

Surgical release forms:

This form MUST be filled out in it's entirety.  All areas for initials and signatures must be completed.  If this is a new patient/pet to us, YOU ALSO MUST FILL OUT THE ADMISSION FORM. Please print and complete the general or routine surgical form:
Routine Surgical Release Form
Routine Surgeries: Form for only Spays & Neuters. If your pet is scheduled for a different or multiple operations (even if a spay/neuter is one of them), please don't fill out this form. Fill out the General form below.
